Eye Care
All Keystone First Community HealthChoices Participants are eligible for routine eye care.* Regular eye exams are important. Call your eye doctor to schedule an eye exam.
What eye doctor do I go to?
If you need help finding an eye doctor, go to Find a Provider and click on Find a Vision Provider. Then call Participant Services to make the request. If you need specialty eye care (for example, treatment of accidental injury), you must go to your primary care practitioner (PCP) first. Your PCP will refer you to a specialist.
What are my eye care benefits?
Participants are eligible for 2 routine eye exams every calendar year. No referral is needed for routine eye exams.
You may have additional eye exams (up to 2 additional exams per calendar year) if the eye doctor completes a form.
